Clinical Pharmacist Services

PCN Clinical Pharmacists help providers and patients achieve better medication outcomes.

The PCN Clinical Pharmacist Service integrates Primary Care Clinical Pharmacists (PCCP) into team-based care to optimize drug theory outcomes for patients. The British Columbia-wide program was developed in partnership with the University of British Columbia’s Faculty of Pharmaceutical Sciences and application to the Burnaby PCN context was led by a Task Group of our physician members, PCN staff, and UBC representatives. 

As part of the primary care team, PCCPs work directly with Primary Care Providers and other team members in the on-going care of patients with complex conditions to:

  1. Prevent and resolve medication-related problems
  2. Educate the patient about their medications
  3. Help remove barriers to patients in following prescription advice

PCCPs use their specialized knowledge to help patients achieve better health outcomes and a higher quality of life. 

PCCPs work in their respective PCNs through a blended service delivery model including: in-office and virtual support to primary care clinics at designated times; referrals for specific cases made by Primary Care Providers; and outreach support to vulnerable populations in the community. 

Our Allied Health professionals can offer services in a variety of languages and have access to interpreter services in over 150 languages.

Eligibility

  • Patient is attached to a Burnaby Family Physician of Nurse Practitioner
  • Patient is 19 years or older

Experience: Michael worked at Vancouver General Hospital where he spent the past eight years as a clinical pharmacist focusing on hospitalist medicine, spinal surgery, and orthopedic surgery. 

Fun Fact: He enjoys trying new recipes with his pressure cooker, jamming on his guitar, painting, and travelling.

Experience: Since the completion of her clinical pharmacy training, she has been working as a clinical pharmacist at Richmond Hospital, primarily in the areas of acute & subacute medicine, and oncology. June has a keen interest in the management of chronic disease, including diabetes and congestive heart failure. 

Fun Fact: In her spare time, she enjoys going for walks with her children, and spending time with her family

Experience: Adam worked at St. Paul’s Hospital and Mount St Joseph Hospital, mainly on the Clinical Teaching Unit (Internal Medicine), Cardiology, High Acuity and Critical Care, and Geriatrics. He also has had experience in outpatient settings such as at the SPH Heart Function Clinic and Atrial Fibrillation Clinic, and most recently in the Vancouver Primary Care Networks on a temporary contract.

Fun Fact: He loves travelling and experiencing new cultures and cuisine at home and abroad. He is also keen to try new craft brews around town.
