Accessing Health Care in Canada

Patient Intake Forms in Multiple Languages

Pathways BC is hosting our fillable patient intake forms on their website. This completed form will help your doctor understand your needs.  

You can download the form, fill it out on your phone, tablets, or desktop, and save it on your device or print it to take to your next health care appointment. Your doctor’s office will be able to translate it back to English using Google Translate or another translation app.
